- Eligibility Requirements -

Child must have a Pediatric Cancer Diagnosis

Child must be <1 years old when diagnosis issued

Child must be within 3 months of last in-treatment appointment

Child's treatment must be administered by Texas Children's Hospital

We understand you are navigating a lot so in hopes to alleviate your burden, please ensure the application is filled out by your medical social worker on your behalf.

We will notify your social worker of application acceptance.
